What I collect
When you book: your first and last name, your email, your phone if you provide it, the language and time zone you chose, your answers to the booking questions, the date and time of the appointment, and the page of the website you came from. When you sign the consent online: your typed name, the city, the date and time of signature and the boxes ticked.
The intake form
If I ask you to fill in an intake form, which may contain health questions, your answers are encrypted as soon as you send it. They are delivered to where I keep my files (my email inbox or my storage space), then erased from this site. They are never kept here: not in the database, not in the backups, not on the screen of my panel.
Why
To organize and follow up your appointments, send you confirmations and reminders, keep your client file and meet my professional obligations. No other use without your explicit consent; commercial offers are sent only if you ticked the box provided, and you can withdraw that consent at any time.
Your consents
Each consent you give is recorded with the exact text you accepted, its version, the language and the time. You can withdraw a consent at any time by writing to info@coachingdv.ca, or through the unsubscribe link at the bottom of messages. The withdrawal applies from then on; it does not undo what was done before.
Where your information is kept
On servers located in Canada (Web Hosting Canada). Appointments are also written to my professional Microsoft 365 calendar, hosted in Canada, along with what you write to me when booking. If you ask me to erase your information, the calendar entry is erased with the rest. Zoom meetings contain none of your personal information. The Cloudflare anti-robot check sees your IP address at the time of the check only; this site does not record it.
Online payments
If you pay online, the payment happens on the Stripe or PayPal page, in my account with them. I never see your card number, and this site never receives it. I keep the amount, the date, the payment reference from the provider and, where applicable, the receipt number. Stripe and PayPal apply their own privacy policy to the payment.
For how long
Your contact details, your answers, my notes and your signed documents are erased 24 months after your last activity with my practice: an appointment, a payment, a package purchase, a receipt, or a consent given or withdrawn. As long as you have sessions left on a package, your file is kept. That is the period my practice chose. The record of your consents remains, without any link to your name. Proof of payment and receipts remain too, without your name or address, for six years, as tax law requires; the PDF of a receipt is deleted as soon as your file is erased. If my practice stops using this service, all your data, receipts included, is destroyed thirty days after the closing; my practice keeps the copy it exported. From the closing on, the intake form no longer accepts answers. An answer you already sent that has not yet reached my practice on the day of destruction is destroyed with everything else.
Your rights
You may ask for access to your information, its correction or its deletion, and withdraw your consent, by writing to info@coachingdv.ca. On request, I give you a full copy of your file, in files you can open with a spreadsheet. You may also file a complaint with the Commission d'accès à l'information du Québec (www.cai.gouv.qc.ca).
Security
Encrypted connection (HTTPS), tokens and signed documents encrypted at rest, access to your file restricted to me alone and logged, daily backups.
Cookies
This site uses only a technical session cookie, needed for the forms to work. No advertising or tracking cookies.
